Submit completed staff change package
For eREB studies and migrated pre-eREB studies:
- Log into the Unity Health eREB to complete and submit a Staff Change form. For information on creating an account and logging in, please see the eREB page. Guides on completing post-approval submissions can be found in the eREB.
For studies that are not yet migrated:
- Complete the staff change form, with an original signature of the Unity Health Lead Applicant and any added or removed Co-Investigators.
- In lieu of an original signature, the submission email can be sent by the PI or by a research team member with the PI cc’d and email confirmations from the Co-Is can be provided.
- Include amended versions all approved documents that are impacted by the change. One clean and one tracked changes version of all amended documents must be provided.
- Submit an electronic copy of the complete submission to researchethics@unityhealth.to, with the subject line “REB <REB Number> – Staff Change”.
Review of staff change package by the Research Ethics Board
- Staff changes are reviewed on a first-come, first-served basis.
- The contact person for the staff change will typically be the Research Ethics Coordinator that worked on the original application. If you don’t have a Research Ethics Coordinator, please email researchethics@unityhealth.to.
- During review of the application the Research Ethics Coordinator will contact you with any clarifications that are needed prior to approving the staff change.
Approval of the staff change application
- Once the review is complete and all concerns have been addressed, your staff change application will be approved or noted to file.
- If an approval letter is issued, it will be sent to you via email.
