Amending Existing Contracts

If you have an existing contract or need to add a work order, please follow the instructions below. If this is a new contract, please click here.

TO AMEND AN EXISTING CONTRACT:

1. Download, fill out, and submit the Excel Contract Intake Form

2. Compile an electronic version of the following documents and email them to researchcontracts@unityhealth.to:

• Copy of the original agreement (if possible)
• External party agreement template (if applicable)
• Completed Excel Contract Intake Form
