Each new research grant at St. Michael’s Hospital requires a new research activity (formerly known as a “cost centre”). If you have a new contract or subcontract or have received a new grant or subgrant, please follow these steps to open a new activity:
Fill out the Research Activity Request Form
Submit the double-sided original version of the completed form to your Research Financial Analyst.
Along with the budget breakdown and funding letter
