Please note that LKS-CHART will not be accepting open submission until January 2017, when Phase 1 of the Enterprise Data Warehouse is operational.
Submit a project proposal
- Complete all sections of the project proposal form.
- Send the project proposal to LKS-CHART by email.
- You will receive a confirmation email that your proposal has been received within 2 business days.
- If you do not receive a confirmation email, please call our Research Coordinator at 416-864-6060 (x76203)
Proposal review
- Each proposal is reviewed by the LKS-CHART team and the St. Michael’s Quality and Performance Steering Committee to determine fit with our core mandate of improving patient outcomes and improving hospital efficiency.
- Division/Department head support for the proposal is verified.
- Review typically takes two to three weeks from receipt of the proposal.
Notification
- Applicants will be notified of the decision by email.
- For proposals that are approved, the LKS-CHART team will follow-up to initiate project planning.
